Direct Debit

If you wish to set up or amend your Direct Debit, please use this form. It must be returned by the 5th of the month before your membership is due. 

This can be returned by email to membership@quiltersguild.org.uk either by a high res scan or a JPEG.  The form must be legible.  If the form is not legible or information is incomplete we will return it and ask for a replacement.

Please complete all sections.  We cannot process this without all the required information including

  • Name and address details of your bank
  • Account holder names
  • Signature
  • Date


Gift Aid

Gift Aid is important to us. If you pay UK Income Tax, tax on interest from savings or investments, or Capital Gains Tax then please would you help us by signing the Gift Aid form here
